Desired Skills/Duties: CAREGIVER POSITION SUMMARY Provides direct care to older adults, including companionship, household assistance and personal care safely within the clients homes. A client may live in a home or apartment in a continuing care retirement community, including independent living, assisted living, and healthcare, or in their own home in the community. This is a PRN position with no guaranteed hours. Hours are based upon the client needs. ESSENTIAL DUTIES AND RESPONSIBILITIES Provides companionship and assistance with household tasks for older adults wishing to maintain independence in their own home. Companionship includes regular interaction, providing comfort, and being responsive to the client s interests, history and emotions. Performs general housekeeping duties including light cleaning, cooking, laundry, and changing linens. Assists clients with personal grooming, dressing, toileting, and bathing. Personal care may involve contact with bodily fluids. Provides transportation as needed to doctor s appointments, outside activities, or shopping, using the client s car, with permission, or a company vehicle. Runs errands on behalf of the client such as banking and shopping. May provide medication reminders and assistance with taking medications according to a plan of care and under the direction of a nurse. Observes and reports any change in the client's condition to the Care Manager. Reports any scheduling or timekeeping issues to the Scheduling Coordinator. Treats clients with respect and dignity, and complies with HIPAA regulations regarding privacy and confidential health information, including establishing and maintaining healthy professional boundaries. Interacts professionally and discreetly with family members, other members of the client s team, other caregivers and staff, and outside entities. Work performed in a continuing care retirement community, such as in a licensed healthcare wing, the Caregiver must abide by the rules and protocols given by the nurses and STNAs of that facility to insure proper patient care. For instance, depending on the license of a unit, the Caregiver may not be allowed to assist with positioning, lifting, or assist with feeding. The Caregiver may be directed to assist a nurse or STNA in some instances. Caregivers in most cases are working alone with a client in his or her own home; therefore, must be able to work independently, make decisions, communicate problems or concerns, and ask for help when needed. Maintains a consistent work schedule and reliable attendance. Completes proper documentation related to services rendered to the client, incident reports, finances, etc. Completes proper documentation related to timekeeping, requesting time off, switching shifts and any other related tasks or documents for personnel or regulatory compliance. MINIMUM JOB REQUIREMENTS Education: High School Diploma or equivalent preferred Licenses or other required certifications: Valid Ohio Driver s license and proof of auto insurance preferred Experience: 6 months previous experience working with older adults in an in-home setting or nursing facility. Specialized knowledge, skills, or abilities: Basic email and phone skills Excellent time management and organizational skills Excellent communication skills Ability to follow directions and work independently Ability to make decisions Ability to multi-task Self-motivator Must be reliable and dependable Must have reliable transportation Professional in manner and appearance, outgoing and passionate personality with exceptional customer service skills Possess discretion and have a respect for confidentiality Ability to be flexible in hours, may need to work early or later hours may need to work early or later hours to accommodate the needs of a client Work may be performed in private homes; therefore, may be exposed to pets, dust, smoke and other irritants.
